Dengue virus NS3 protein localizes to the nucleus in the early infection phase (8h-12h) (Palacios-Rápalo et al., 2021; Gandhi et al., 2023). The process requires importin-alpha and can be inhibited in vitro and in mice, reducing Dengue infection in mice (Palacios-Rápalo et al., 2023). In a clinical phase 2/3 trial (N=203), ivermectin accelerated NS1 antigenemia clearance in symptomatic dengue patients. However, clinical efficacy of ivermectin was not observed, with the cited study suggesting factors such as patient heterogeneity, timing of drug administration, and dosage as potential reasons for this outcome (Suputtamongkol et al., 2021).
