The SLC12A3 gene encodes the thiazide-sensitive sodium-chloride cotransporter. In the distal convoluted tubules of the kidney, SLC12A3 is the major mediator of sodium and chloride reabsorption in the apical membrane of the mammalian distal convoluted tubule and contributes to general electrolyte homeostasis in the body.

Distal convoluted tubule diuretics are primarily used to treat hypertension (Wright et al. 2018). They inhibit SLC12A3 causing the body to release NaCl and water into the lumen, thereby increasing the amount of urine produced each day. This reduces water load in the body, contributing to the lowering of blood pressure and swelling (Winer et al. 1961, review Duarte & Cooper-DeHoff 2011). Thiazide diuretics (eg. chlorothiazide, cyclothiazide, diazoxide) and thiazide-like diuretics (eg. chlorthalidone, metolazone) bind to and inhibit SLC12A3. The proposed target for the candidate drugs listed here is also thought to be SLC12A3. Hydrochlorothiazide is the prototypical thiazide diuretic drug.
